Community Health Choice is committed to ensuring that visitors to our website understand our privacy practices. This Privacy Statement explains: (1) what personal information about you may be collected; (2) how personal information about you will be used; (3) who may have access to personal information about you; and (4) how we protect your personal information within our secure website. This privacy statement applies only to this website and the information collected on this website.

We will protect confidential personal information you may share with us when you visit our website. We will not collect any personal information  about you unless you provide that information to us voluntarily. If you do provide us with personal information, we will not sell, license or transmit that information outside of Community unless you authorize us to do so or we are required or permitted to do so by law. In addition, we may disclose information to companies that we have contracted with in order to assist us in administering or providing services for our health benefit plan. If we disclose confidential personal information, it will be in compliance with applicable laws and regulations, and we will require the recipient to protect the information and use it only for the intended purpose.

Personally identifiable information means data that is unique to an individual, such as name, address, telephone number, Social Security number, Member or Provider ID number or date of birth. From time to time, we may request personal information, including personally identifiable information, from you through our website. We request this information in order to deliver materials to you or to respond to your questions. Members and Providers in Community’s network have the ability to register in a secured area. This registration allows us to tailor the materials we send to you.

Important Marketplace Emails: If the Marketplace has your email address, they’ll automatically send you important information, updates, and reminders about Marketplace enrollment. You can opt out of these communications at any time. To do this, click on the “unsubscribe” link in the footer of any Marketplace email.

Privacy and the use of your information: The Marketplace will keep your information private as required by law. Your answers on your Marketplace application will only be used to determine eligibility for health coverage or help paying for coverage. The Marketplace will check your answers using the information in their databases and the databases of other federal agencies. If the information doesn’t match, the Marketplace may ask you to send them proof. The Marketplace won’t ask any questions about your medical history. Household members who don’t want coverage won’t be asked questions about citizenship or immigration status.

As part of the application process, the Marketplace may need to retrieve your information from the Internal Revenue Service (IRS), Social Security, the Department of Homeland Security (DHS), and/or a consumer reporting agency. They need this information to check your eligibility for coverage and help paying for coverage if you want it and to give you the best service possible. The Marketplace may also check your information at a later time to make sure your information is up to date. The Marketplace will notify you if they find something has changed.

  • I agree to have my information used and retrieved from data sources for this application. I have consent for all people I’ll list on the application for their information to be retrieved and used from data sources.
  • I understand that I’m required to provide true answers and that I may be asked to provide additional information, including proof of my eligibility for a Special Enrollment Period if I qualify. If I don’t, I may face penalties, including the risk of losing my eligibility for coverage.

You may email us with any question or comment you have concerning Community. Community handles all email correspondence with personal information confidentially. Once we receive your email, we will assign it to the most appropriate person capable of handling your question. We will not share your email with anyone who is not involved in answering your question.

Please note that email correspondence on the internet may pass through public and private networks and may be accessed and viewed by other internet users without your knowledge or permission. For this reason, we recommend that you do not send us any confidential information via email. We are not responsible for the security or confidentiality of information that you send to us through the internet using email communications. In order to keep improving our website, we collect certain information in aggregate from the users of our website. This information does not identify you personally but allows us to determine the total number of people visiting our site and what pages they are most often visiting. Using this information, Community can determine which pages receive the most Internet traffic, and that helps us make our site as user-friendly as possible.

We also collect information on our website through the use of “cookies.” A cookie is a small element of data that we can transfer to your computer’s hard drive that tells us if you are a repeat user, what pages best serve you, and allows you easier access when you return to our site. The cookie does not track any personal information about you such as name, address or telephone number. You may request that we limit the collection, creation, disclosure, access, maintenance, storage and use of your Personally Identifiable Information for the sole purpose of our assisting you in applying for health insurance or obtaining an eligibility determination, facilitating payment for your first premium, assisting you in updating or canceling your enrollment in a health insurance plan, and for performing other authorized functions specified in our agreements with CMS. Community is committed to making sure that the information you share with us is confidential and secure. We protect your information using Secure Socket Layers (SSL), a process that allows only authorized personnel to access your personal information. All authorized personnel must abide by Community’s security and confidentiality policies.

Our website may contain links to other websites not owned or controlled by Community. These links are provided as a courtesy to our website visitors. Community is not responsible for the content or privacy practices of other websites. We encourage you to review the privacy policy of any website visited.
