Limited Deadline Extension

To make sure that you get covered, Community Health Choice has extended the deadline to pay your premium for January coverage through this Friday, January 13.

It's important to us that you get the health coverage that you signed up for, and it's quick and easy to pay your premium online.

If you are a new Member with coverage starting in January, you must make this payment by midnight of this Friday. If you do not, you will be dis-enrolled from your Community plan and will not have health insurance coverage for January. If this happens, you will only have until January 31 to re-enroll for 2017 coverage.

Provide information

You will need your Exchange Subscriber ID to make an online payment. You can find this number on your first premium invoice, or you can use your Social Security Number to look it up by selecting the "where do I find this" link on our payment page.

If you are unable to make a payment, please call our Member Services department as soon as possible at 713.597.5564, or toll-free at 1.855.315.5386. Our Member Services department is available by phone from 7:00 a.m. to 7:00 p.m., Monday through Friday.
